Blood Collection Instructions
1) Fill Out Test Request Form
2) Fill Out the AdvanceDx100 Serum Separator Card
3) Wash Your Hands
4) Nick Your Finger
Remove the protective blue tip from the Lancet
With your hand on the table, Palm side up, place the tip of the Lancet slightly off center
Hold Lancet steady, press down firmly until it "clicks", then release.
5) Place Blood Drops in the box to the left on your card
6) 3-4 VERY LARGE drops will usually be enough to reach the 1st line.
THE STRIP MUST TURN RED TO THE 1st LINE.
7) Allow to Dry 15 Min
8) Keep Your Portion of Form
9) Mail Sample to the Lab
